To apply to become a member of the Police and Clergy Team, you must fill out an application and return it to the Houston Police Department. 

The following items must be included in your application package:

1. Fingerprints - To be taken at Houston Police Headquarters - 1200 Travis.  Please contact PACT liaison for scheduling information.
2. Application
3. Termination of Service Agreement
4. Volunteer Agreement
5. Military Release
6. Personal Info Release
7. ID Info Sheet
8. Acknowledgment Waiver and Release 
9. Copy of Social Security Card
10. Copy of Texas Drivers License
11. Contact Sheet Information

The following will automatically disqualify you from participation in the Police and Clergy Team

1. Offenses against children - There is no time time limit on arrests or convictions.
2. Drugs - Selling within last 10 years. 
3. Robbery - Within last 15 years.
4. Theft - Within last 10 years.
5. Murder - No time limit.
6. Assault with a deadly weapon - Within last 10 years.
7. Drug use and/or possession - Within last 10 years.
